Licensed Practical Nurse (LPN)

What You Will Do

• Provide skilled nursing care to patients in their homes
• Follow the physician approved plan of care
• Monitor patient conditions and report changes
• Educate patients and families on medications and care plans
• Document visits accurately and timely
• Work closely with an interdisciplinary care team

What You Need

• Current Licensed Practical Nurse (LPN) license in the state of practice
• Minimum 1 year experience in acute care or similar setting
• CPR certification through American Heart Association or American Red Cross
• Strong communication and patient care skills
• Reliable transportation for patient visits
• Commitment to providing high quality care
