Licensed Practical Nurse (LPN) – Home Health
📍 Full-Time | Monday–Friday

Deliver skilled nursing care under RN/physician direction
Follow and implement individualized patient care plans
Monitor patient conditions and report changes promptly
Educate patients and families on self-care and safety
Administer medications and maintain accurate documentation
Collaborate with RNs and interdisciplinary care teams
Help prevent avoidable hospitalizations through proactive care
Active LPN/LVN license (state required)
Graduate of an accredited nursing program
1+ year of clinical nursing experience
Home Health or Hospice experience (preferred)
Ability to travel throughout the East Hampton coverage area (~50%)
Reliable transportation, valid driver’s license, and auto insurance
Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs
