Licensed Vocational Nurse LVN | Home Health

📍 Full-Time | Flexible Schedule | 1:1 Patient Care

Locations: Baytown, Deer Park, South Houston, La Porte, Pearland

What You’ll Do

• Provide skilled nursing care in patients’ homes
• Monitor patient conditions and report changes to the care team
• Administer medications and treatments as ordered
• Educate patients and families on care plans and disease management
• Complete accurate and timely documentation
• Collaborate with RNs, physicians, and interdisciplinary care teams
• Build trusted relationships with patients and caregivers

What You’ll Need

• Current LVN/LPN license in the state of practice
• Minimum of 1 year of nursing experience in acute care or similar setting
• CPR certification through American Heart Association or American Red Cross
• Strong communication and time management skills
• Compassionate, patient-focused mindset
• Reliable transportation, valid driver’s license, and current auto insurance
• Ability to frequently lift, push, pull, and support up to 50 pounds
