Home Health RN Case Manager

What You'll Do

As a Home Health RN, you will:

  • Provide skilled nursing care to patients in their homes

  • Perform comprehensive patient assessments and develop individualized plans of care

  • Monitor patient progress and adjust care plans as needed

  • Educate patients, families, and caregivers on disease management, medications, treatments, and safety practices

  • Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate care

  • Complete timely and accurate clinical documentation

  • Identify changes in patient conditions and communicate findings to the care team

  • Participate in case conferences and interdisciplinary care planning

  • Ensure compliance with all regulatory, agency, and professional nursing standards

  • Maintain patient confidentiality and HIPAA compliance at all times

What You'll Need
  • Current, unrestricted Registered Nurse (RN) license in the state of practice

  • Minimum of one (1) year of nursing experience in a clinical care setting

  • Strong assessment, critical thinking, and communication skills

  • Ability to work independently and effectively manage a patient caseload

  • Reliable transportation, a valid driver's license, and current auto insurance

  • Willingness to travel throughout the assigned service area

  • Physical Requirements

    Sit, stand, bend, lift, and move intermittently, and be able to lift 50-100 lbs

Preferred Qualifications

  • Home Health experience

  • Hospice experience

  • Experience with HomeCare HomeBase (HCHB)

  • Associate's or Bachelor's Degree in Nursing
