Home Health Clinical Team Manager (RN)Lead the Team Behind Exceptional Home Health Care
The Clinical Team Manager (RN) is the clinical leader responsible for supporting and guiding our field clinicians while ensuring patients receive safe, high-quality, coordinated care in the place they call home.

Provide day-to-day leadership, support, and clinical oversight to a multidisciplinary team including RNs, LPNs/LVNs, CNAs, therapists, and other direct care clinicians.
Ensure the delivery of safe, effective, and patient-centered care throughout the home health episode of care.
Coordinate patient assignments, referrals, and care transitions to promote continuity and timely service delivery.
Oversee the development, implementation, and ongoing management of individualized patient care plans.
Review and approve plans of care and evaluate clinical appropriateness of proposed changes.
Coordinate physician orders and ensure timely communication between clinicians, providers, patients, and caregivers.
Ensure patients are admitted and services are initiated within regulatory and organizational timeframes.
Conduct regular clinical documentation and OASIS reviews to support quality outcomes and regulatory compliance.
Monitor key clinical and operational metrics and collaborate with branch leadership to drive performance improvement initiatives.
Provide coaching, mentorship, and professional development opportunities to field clinicians.
Facilitate regular team meetings and maintain strong communication across disciplines.
Participate in recruitment efforts, candidate interviews, onboarding, and staff engagement initiatives.
Support clinician education, competency validation, and ongoing training efforts.
Remain available for patient visits and field support as operational needs require.
Promote a culture of collaboration, accountability, safety, and clinical excellence.
Perform additional responsibilities and special projects as assigned.
Current, unrestricted RN license in the state of practice.
Associate Degree in Nursing required; Bachelor's Degree in Nursing preferred.
Minimum of two years of home health experience as a Registered Nurse in a Home Health or Hospice setting.
Previous leadership, supervisory, preceptor, or team lead experience preferred.
Strong clinical assessment, critical thinking, and decision-making skills.
Ability to effectively coach, mentor, and support clinicians in a fast-paced environment.
Experience with OASIS documentation and home health regulations preferred.
Ability to quickly become proficient in enterprise systems including HomeCare HomeBase, Workday, and Microsoft Office applications.
Willingness to travel throughout the branch coverage area as needed to support patients and staff.
Valid driver's license, reliable transportation, and current auto insurance in accordance with state requirements.
Ability to occasionally lift, push, pull, and support up to 50 pounds, including assisting with patient transfers and moving equipment when necessary.
