• Monday–Friday Schedule

Enjoy a consistent weekday schedule with 30 patient visits per week, helping you maintain a healthy work/life balance.

• Localized Territory

Care for patients throughout Kingwood, Humble, New Caney, Dayton, and Liberty, allowing you to build meaningful relationships within your community.

• Shared On-Call Rotation

Participate in approximately one weekend of on-call coverage each month, allowing for predictable scheduling while supporting our patients when they need us most.

• One-on-One Patient Care

Focus on one patient at a time, providing personalized, compassionate care in the comfort of their home.

As a Home Health LVN, you'll provide skilled nursing care under the direction of an RN Case Manager while helping patients safely recover and remain independent in their homes.

Your responsibilities will include:

• Deliver skilled nursing visits in accordance with the physician's orders and individualized plan of care.

• Monitor patient conditions and promptly communicate changes to the RN Case Manager and interdisciplinary team.

• Provide wound care, medication management, treatments, and patient education.

• Reinforce disease management and self-care techniques with patients and caregivers.

• Collaborate closely with RN Case Managers to ensure high-quality, coordinated care.

• Document patient assessments, interventions, medications, and progress accurately and timely.

• Promote patient safety while helping prevent avoidable hospitalizations and emergency department visits.

• Build trusting relationships with patients, families, physicians, and care team members.

• Maintain compliance with agency policies, HIPAA regulations, and clinical standards.

• Graduate of an accredited Practical Nursing program

• Current, unrestricted LVN license

• Minimum of one (1) year of nursing experience in a clinical setting

• Home Health or Hospice experience preferred, but not required

• Ability to travel throughout the assigned territory

• Reliable transportation, valid driver's license, and current auto insurance

• Strong communication, organizational, and time management skills
