As a Licensed Vocational / Practical Nurse LPN / LVN you’ll contribute to our success in the following ways:
- Follow each patient's individualized care plan and physician orders.
- Monitor patient progress and report changes or safety concerns to the care team.
- Help patients and caregivers learn self-care techniques to support their health.
- Work with the RN Case Manager to update care plans as patient needs change.
- Administer and document medications according to agency guidelines.
- Communicate with physicians, patients, families, and the care team to ensure coordinated care.
- Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
- Deliver compassionate, high-quality care that supports positive patient outcomes.
What is Required?
- Current State License as a Licensed Vocational / Practical Nurse
- Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
- 1 year of experience in a clinical care setting
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Experience in a hospice or home health environment is preferred
