What You'll Do

• Travel to assigned Elara Caring locations to provide home health physical therapy.
• Evaluate patients, develop treatment plans, and help them regain mobility and independence.
• Collaborate with local clinical teams to deliver exceptional patient care.
• Document patient progress and support continuity of care.

What You'll Need

• Degree from an accredited Physical Therapy program.
• Active PT license or ability to obtain licensure in assigned states.
• At least one year of clinical experience.
• Willingness and ability to travel 100% to assigned locations.
• Valid driver's license and strong communication skills.
