Banner has a specific cover page. Please ensure candidates clearly outline ALL criteria they’re willing to accept. The more flexible they are with locations, shifts, and departments, the faster we can secure an offer!
· Candidates must be able to start on the listed start date, or no more than one week later
· RTO must be on the cover page and cannot exceed 7 days. No RTO during the first week of assignment
· Please list all facilities the candidate is willing to work in. Specify facility name(s), states, or all, if open to any facility in any state
· All shifts the candidate is willing to work
· All departments the candidate is experienced and willing to work
All candidates submitted should be pre-closed and ready to accept. Please let us know immediately if the candidate withdraws or is no longer available.
Submission Requirements
· Must have a minimum of 24 months ICU experience (with recent experience in last 12 months) - [REQUIRED]
· Life support certifications must be from AHA, ARC, or ASHI - [REQUIRED]
· No hits on state license to practice - [REQUIRED] [DOCUMENT]
· RTO cannot exceed 7 days, No RTO during the first week of assignment - [REQUIRED]
· Candidate driver’s license to verify residency - [REQUIRED]
· Travel experience - [HIGHLY PREFERRED]

· Active BLS certification from approved vendor [REQUIRED] [DOCUMENT]

About the Facility
· Banner assesses a 2% payment discount fee and a 2% technology fee on all transactions.
· Learning modules must be completed prior to start of assignment.
· Non-Billable hours have been adjusted to reflect 16 hours for each assignment.
· Client or Vendor may cancel an assignment in progress with one full week’s notice without penalty.
