Physical Therapist (PT)

Job Summary

The Physical Therapist evaluates and treats patients of all ages, developing and modifying individualized plans of care based on patient response, clinical findings, and available evidence. The PT serves as a role model in the delivery of professional services and acts as a clinical resource for staff and students.

Essential Functions

  • Evaluate and treat patients of all ages and modify the plan of care as appropriate.
  • Perform patient transfers with assistance as needed from bed, chair, and floor.
  • Demonstrate safe and appropriate body mechanics to reduce the risk of injury to patients, staff, and self.
  • Treat patients and colleagues with dignity, respect, empathy, and compassion.
  • Develop an appropriate plan of care integrating goals, treatment interventions, outcomes, and discharge planning.
  • Adjust treatment approaches based on patient response and current evidence.
  • Educate patients, families, caregivers, students, and other healthcare providers using effective teaching methods.
  • Evaluate clinical findings to determine physical therapy diagnoses and expected outcomes.
  • Perform comprehensive examinations using reliable and valid methods relevant to the patient's chief complaint, screening results, and medical history.
  • Complete documentation in accordance with clinical, regulatory, and organizational standards.
  • Screen patients to determine the effectiveness of and need for therapy services when appropriate.
  • Utilize support personnel according to legal standards, ethical guidelines, and organizational policies.
  • Monitor care delivered by support personnel and provide regular feedback.

Department-Specific Responsibilities

  • Evaluate and implement intervention plans using functional activities that address the physical and emotional needs of patients and families.
  • Modify treatment approaches based on patient behavior, response, and performance.
  • Utilize standardized outcome measures to accurately assess and document functional performance.
  • Complete all required documentation in accordance with regulatory and organizational standards.
  • Collaborate with patients, families, and significant others throughout the rehabilitation process.
  • Provide patient and family education and training as necessary.
  • Provide supervision to Physical Therapist Assistants (PTAs) and rehabilitation technicians in accordance with applicable regulations.
  • Communicate clearly with the medical team regarding:
    • Patient performance
    • Changes in patient status
    • Activity and transfer recommendations
    • Discharge disposition
    • Equipment needs for the next level of care

Education

Required:

  • Bachelor's Degree in Physical Therapy.

Required Certifications & Licensure

  • Active Illinois Physical Therapist (PT) License
  • BLS – American Heart Association (AHA)
