About the Opportunity

We are seeking a motivated, compassionate Physical Therapist (PT) to join a PACE (Program of All-Inclusive Care for the Elderly) therapy team. PACE is a unique, community-based model - distinct from a SNF, ALF, or ILF - that delivers fully coordinated medical and supportive care so frail, nursing-home-eligible older adults can keep living safely in their community.

This role is ideal for a clinician who values relationship-driven geriatric care, true interdisciplinary collaboration, and hands-on, 1:1 treatment.

Job Summary

Evaluate and treat participants' mobility and motor function for functional performance. Provide goal-directed, activity-oriented treatment to promote optimum independence in self-care, functional mobility, and daily life tasks. Ensure continuation of progress through effective education of the participant, family, or support system, and provide adaptive methods and equipment as appropriate. Participate in family/participant care-plan meetings, the interdisciplinary team (IDT), and committees as determined by the Director of Therapy.

What You'll Do

  • Plan, manage, and coordinate participant evaluations and treatments based on age-specific needs
  • Evaluate the whole participant — incorporating medical-record data to assess functional status, physical disability, movement dysfunction, pain, community mobility, and rehabilitation potential
  • Correlate evaluation data to determine the IDT plan of care, directed toward short- and long-term goals
  • Administer skilled, 1:1 treatment focused on posture, locomotion, strength, endurance, cardiopulmonary function, coordination, joint mobility, flexibility, pain, and healing/repair
  • Determine appropriateness of each participant's response to treatment and consult with the physician and team on care-plan modifications
  • Analyze tasks and adapt the environment, tools, materials, and treatments to participant needs and socio-cultural context
  • Participate in IDT conferences and family meetings as a team player ensuring continuity of care
  • Schedule participants according to caseload and demand, coordinating with other services
  • Delegate treatment to support personnel per their expertise while retaining responsibility for the participant and treatment program at all times
  • Follow all documentation and form-completion policies and procedures

A Typical Day

Your day blends 1:1 treatment sessions, interdisciplinary team meetings, and family meetings — balancing direct hands-on care with the coordination and communication that make the PACE model work.

  • Bright, spacious second-floor therapy gym with large windows
  • Collaborative multidisciplinary environment (PT, OT, SLP, nursing, primary care, social work, dietary, and more)
  • Indoor, climate-controlled setting with occasional travel/outdoor exposure
  • Supportive leadership and administrative support

  • Excellent verbal, written, interpersonal, and customer-service skills
  • Excellent organizational skills and attention to detail
  • A caring attitude toward participants and their families
  • Strong time-management, analytical, and problem-solving skills with a proven ability to meet deadlines
  • Ability to function well in a fast-paced and at times stressful environment
  • Proficient with Microsoft Office Suite or related software

Qualifications

  • Active, unrestricted state Physical Therapist license
  • Current CPR/BLS certification
  • High school diploma or equivalent
  • Experience with geriatric, frail, or medically complex populations preferred

Physical Requirements

Safety-sensitive role. Constant standing, balance, grasping, and full-body range of motion; occasional walking (up to ~0.25 mile), sustained bending/squatting/kneeling, reaching overhead, lifting up to 100 lbs floor-to-waist, and push/pull up to 100 lbs of force. May be exposed to blood/body fluids, communicable diseases, and other clinical environmental conditions.
