Job Details:
Job Title: RN: Case Manager
Location: Colorado Springs, CO
Setting: Home Health
Pay Range: 2190.60 - 2300.13
Schedule: M-F 8a-5p
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: Current, unrestricted state Registered Nurse license; Associate or bachelor's degree in nursing or related field; Ability to analyze complex medical information and make rational decisions; Case Management certification.
Responsibilities:
- Perform PRN/focused and routine assessments of high risk members.
- Collaborate with primary care providers to develop and implement individualized care plans.
- Address, identify, and reassess appropriate levels of care, including non-medical support to ensure treatment compliance.
- Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals.
- Keep detailed records of clinical, functional, and fiscal outcomes during the management process.
