Description
Start Date: ASAP
Location: Northern California
Compensation: Details provided to qualified applicants
Schedule: Every other weekend required; caseload up to 25 patients; no call required; floating based on departmental needs

Requirements:
- Minimum 2 years of acute care hospital case management experience (must be current within the last year)
- Active RN license in good standing
- Proficiency with Epic electronic health records system
- Experience with HMOs, IPAs, and managed care processes
- Knowledge of advance directives, POLST, and protective services processes
- Strong skills in discharge planning, appeals and denials, care coordination, and DRG assignment
- Understanding of CMS guidelines, HIPAA regulations, and Joint Commission standards
- Ability to float between units based on departmental needs
