Schedule: Every other weekend required; caseload up to 25 patients; no call required; floating based on departmental needs

Requirements:

• Minimum 2 years of acute care hospital case management experience (must be current within the last year)
• Active RN license in good standing
• Proficiency with Epic electronic health records system
• Experience with HMOs, IPAs, and managed care processes
• Knowledge of advance directives, POLST, and protective services processes
• Strong skills in discharge planning, appeals and denials, care coordination, and DRG assignment
• Understanding of CMS guidelines, HIPAA regulations, and Joint Commission standards
• Ability to float between units based on departmental needs
