Schedule: Monday through Friday, 8:00 AM to 4:30 PM

Requirements:

• Active California RN license in good standing
• Current BLS certification through the American Heart Association
• Minimum 2 years of recent RN Case Management experience in a hospital setting
• Minimum 2 years of recent discharge planning experience in a hospital environment
• Strong assessment, care planning, and care coordination skills
• Ability to work collaboratively with multidisciplinary teams and external review agencies
