Location: Portland, Oregon

Schedule: Caseload management with independent home visits

- Plan, organize, and direct comprehensive home care services for patients in their residences
- Manage your own patient caseload with autonomy and clinical independence
- Collaborate with interdisciplinary care teams to achieve optimal patient outcomes

Requirements:
- Active RN license in Oregon
- Home health or community nursing experience preferred
- Strong clinical assessment and care coordination skills
- Ability to work independently and manage time effectively
- Valid driver's license and reliable transportation
