Hospice Registered Nurse Qualifications

• Minimum 1 year of recent hospice RN experience (within the past 3 years)
• Proficiency with HCHB (Homecare Homebase) – required
• Experience with OASIS documentation – required
• Active Wisconsin or Compact RN License (must be in hand at time of submission)
• Current BLS certification – required

Hospice RN Responsibilities

• Provide compassionate hospice nursing care to patients in homes and facilities throughout Rhinelander, WI
• Complete accurate and timely clinical documentation using HCHB and OASIS systems
• Work collaboratively with interdisciplinary healthcare professionals to develop and implement individualized care plans
• Educate patients and families about hospice services, symptom management, and end-of-life care options
• Deliver skilled nursing care, pain and symptom management, and emotional support to patients and families
• Ensure compliance with regulatory and organizational standards for hospice nursing jobs
