Job Opportunity: RN Hospice Case Manager
Position Details
- Employment Type: Contractor
- Contract Length: 13 weeks
- Location: Servicing Sheboygan and Manitowoc Counties
- Patient Population: Adults only – End of Life patients
- Caseload: 13-15 patients (home, assisted living facility, skilled nursing facility)
- Patient Ratio: 1:1
Job Description
- Manage hospice patients in home and facility settings
- Perform admissions and coordinate care
- Provide end-of-life care and planning
- Conduct wound assessments
- Manage pain and symptoms
- Perform triage as needed
- Use EPIC Remote Client for documentation (required)
- Utilize IV pumps (Sapphire)
Schedule & Orientation
- Shift: Monday to Friday, 8:00 AM – 4:30 PM
- Weekend rotation: One weekend per month
- Call: One call shift per month
- Holidays: No holiday shifts required
- Orientation: Up to 1 week, based on comfort level
- Scrub color: Any color or business casual attire
Required Qualifications
- Valid RN license in Wisconsin
- Minimum 2.5 years of case management experience, including within the last year
- Associate of Science in Nursing / Associate Degree in Nursing
- BLS certification
- Experience working with adult patients
- Experience in a home health setting
- Hospice experience
- Experience with EPIC Remote Client
- Care coordination skills
- Triage skills
- Travel experience
Additional Information
- Floating: May be required anywhere within the counties served
- Support Team: RNs, Case Managers, Manager on call
- Time off requests approved by manager
Travel Experience
- First-time travelers accepted if they have hospice and EPIC Remote Client experience
- No explicit prior travel experience required
