Job Requirements (Social Work Qualifications)
- Master’s Degree in Social Work (MSW) — Required for CA and HI assignments
- Minimum 1 year of experience in an acute or long-term medical setting (clinical social work, hospital social worker, discharge planner)
- 2 years of acute or long-term medical setting experience preferred
Key Responsibilities (Case Management & Clinical Social Work)
- Collaborate with interdisciplinary teams to develop and facilitate safe, patient-centered discharge plans for post-hospice or post-inpatient care
- Assess patients, families, and caregivers to determine appropriate levels of hospice care and refer to outpatient resources and community supports as needed
- Initiate, document, and reassess Plans of Care (POC) and provide patient / family teaching based on physical, emotional, and psychosocial needs
- Conduct comprehensive psychosocial assessments, considering clinical status, mental health, family dynamics, and community resources
- Provide care coordination, resource linkage, and follow-up to promote safe transitions of care
- Perform other job-related duties as assigned
