Job Details

• Location: Orange, CA 92868 (Orange County) – California social work jobs, close to beaches and Southern California attractions
• Setting: Inpatient Cardiology/LVAD Unit – cardiac social work, inpatient case management
• Contract Length: 13 weeks to start, with likely extension up to 6 months (travel contract)
• Start Date: Around May 18, 2026 (flexible by one week)
• Schedule: 5 shifts per week, 8-hour day shifts (40 hours/week)
• Weekly Estimated Pay: $2,260 – $2,430

Qualifications / Job Requirements

• Active California Licensed Clinical Social Worker (LCSW) required — California LCSW licensure for inpatient social work
• Minimum 1 year experience in case management or related social work (medical social work preferred)
• Experience with cardiology, LVAD (ventricular assist device), or heart failure patients preferred — Cardiac social worker experience a plus
• Strong communication, organizational, and problem-solving skills; experience with interdisciplinary team collaboration
• Experience in discharge planning, care coordination, community resource linkage, and clinical documentation (EMR)

Responsibilities / Key Duties

• Provide comprehensive case management and psychosocial assessments for cardiology/LVAD inpatients
• Develop and coordinate discharge plans, link patients to community resources, and ensure safe transitions of care
• Advocate for patient needs and provide family support for complex cardiac and LVAD care
• Collaborate with physicians, nurses, and allied health professionals as part of an interdisciplinary team to optimize patient outcomes
• Perform crisis intervention, patient education, and care coordination for heart failure and LVAD populations
• Maintain accurate, timely clinical documentation in compliance with hospital policies and regulatory standards
