Required Qualifications

• Active California Licensed Clinical Social Worker (LCSW) required — California social work license

• Excellent communication, organization, and interdisciplinary collaboration skills

Primary Responsibilities

• Provide comprehensive case management and social work services for patients on the inpatient psychiatric unit
• Conduct biopsychosocial assessments and develop individualized treatment and discharge plans
• Coordinate care with psychiatrists, nurses, therapists, and community providers to ensure continuity of care
• Facilitate timely discharge planning, community referrals, and linkage to outpatient mental health resources
• Advocate for patient needs, complete utilization reviews, and assist with behavioral health authorizations as needed
• Document assessments, treatment plans, and progress notes accurately in the EMR to meet compliance standards
