Position: Travel RN - Case Management / RN Case Manager (Discharge Planning)
Employment Type: Travel Contract (Travel Nursing)
Location: On-site in Portland, OR (97239) — Portland healthcare job / Oregon nursing job
Contract Duration: 13 weeks
Hours per Week: 40
Shift: Day shift (7:00am - 3:30pm), 8-hour days
Shifts per Week: 5 (Monday-Friday with rotating weekends)
Estimated Weekly Pay: $2,104 - $2,338
Start Date: April 6, 2026
EMR: Epic (Epic EMR experience preferred)

Required Qualifications

Active Registered Nurse (RN) license in Oregon or compact nursing license eligible
Minimum 1 year recent case management or discharge planning experience
Experience documenting in Epic EMR preferred
Strong communication, prioritization, and organizational skills
Ability to collaborate effectively with physicians, social workers, care coordinators, and community resources

Key Responsibilities

Lead discharge planning and care coordination to ensure safe, timely transitions from hospital to home or post-acute care
Assess patient needs, arrange services, and coordinate authorizations and transportation for discharge
Collaborate with multidisciplinary teams including physicians, nursing staff, social work, and case management
Provide patient and family education on post-discharge care plans, resources, and follow-up
Document case management interventions and discharge summaries in Epic
Provide patient advocacy and support throughout the inpatient stay and discharge process
