The RN Home Health Case Manager is responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with medical regime.

Minimum Requirements:

• 2 Years Case Management experience preferred in acute care setting or home health setting with previous discharge planning experience
• Active OR RN License or pending license
• BLS
• Current CPR if applicable
• TB questionnaire, PPD or chest x-ray if applicable
• Current Health certificate (per contract or state regulation)
• Must meet all federal, state and local requirements
• Must be at least 18 years of age
