- Setting: Acute Care Hospital – inpatient case management
- Duration: 26 weeks
- Shifts per Week: 5
- Shift Duration: 8-hour days (08:00 – 16:30)
- Schedule: Day shift, every other weekend required
- Weekly Estimated Pay: $2,432 - $2,703
- Start Date: May 11, 2026
Job Requirements
- Active California Registered Nurse (RN) license required
- BLS certification (AHA) required
- Minimum 5 years of RN case management experience with proven success in inpatient acute hospital case management
- Strong experience in discharge planning, utilization management, and care coordination
- Experience with Epic EHR and Milliman (utilization review) preferred
- Excellent communication, documentation, and critical thinking skills
- Comfortable working with diverse patient populations and community resources
- Emergency department case management experience is a plus
Primary Responsibilities
- Perform initial and ongoing patient assessments including medical, psychosocial, and financial needs
- Develop and implement patient-centered, cost-effective care plans and individualized discharge plans
- Identify readmission risks and coordinate support services to reduce avoidable readmissions
- Ensure patients meet level-of-care criteria and actively monitor length of stay
- Collaborate with physicians, nursing staff, social work, utilization management, and community providers
- Coordinate transitions of care and post-discharge resources with families and community agencies
- Address barriers to discharge and manage complex/high-utilizer cases
- Ensure compliance with Medicare, CMS, Joint Commission, and other regulatory requirements
- Participate in interdisciplinary rounds, case conferences, and process improvement initiatives
- Document consistently and thoroughly in EHR systems (Epic experience highly valued)
