Position Details – Homecare RN (Travel)

• Weekly Estimated Pay: $2,120 - $2,230 (competitive travel nursing pay)
• Location: Forty Fort, PA 18704 (Luzerne County) — Pennsylvania nursing jobs
• Employment Type: Travel / Home Health RN
• Hours per Week: 40 (Guaranteed minimum 32 hours/week)
• Shift: 8-hour shifts, Monday–Friday (8:00am–4:30pm)
• On-Call/Weekend: Every 3rd weekend; 1 night of on-call per week
• Duration: 39 weeks (through December 31, 2026)
• Dress Code: Scrubs (no specific color required)
• Mileage: Reimbursed between patient visits

Job Requirements / Qualifications

• Active Registered Nurse (RN) license in Pennsylvania
• Associate's Degree in Nursing (minimum education requirement)
• At least 1 year of nursing experience; recent Home Health experience preferred
• Experience with HCHB (Homecare Homebase) is a plus
• Professional certification required

Key Responsibilities – Home Health RN & Case Manager

• Serve as RN case manager for a designated territory within Luzerne County
• Provide skilled nursing visits, recertifications, discharges, and start/resumption of care
• Deliver 100% patient-focused care to ensure high-quality home health outcomes
• Collaborate with other case managers and clinical staff to provide coverage and continuity of care
• Meet productivity expectations and maintain accurate, compliant documentation
