Monday–Friday | 40 hours/week | No hospital shifts

Step Out of the Hospital Without Leaving Patient Care

Are you an acute care RN ready for more autonomy, meaningful patient relationships, and control over your schedule?

This Home Health Case Manager RN role is designed for nurses coming from ICU, ED, or Med/Surg who want to continue working with patients — but in a more independent, one-on-one environment outside the hospital.

Instead of juggling multiple bedside assignments, you'll focus on individual patients, helping them safely transition from hospital to home while managing and coordinating their care.

You’ll still use your critical thinking and clinical expertise, but with greater flexibility, self-scheduling, and improved work-life balance.

What You'll Do

• Provide one-on-one care management for patients transitioning from hospital to home
• Assess patients with acute and complex medical conditions in their home environment
• Develop and manage individualized care plans
• Coordinate with physicians, therapists, and interdisciplinary teams
• Educate patients and families on medications, treatments, and recovery plans
• Monitor patient progress and adjust care plans to optimize outcomes

Qualifications

• Active Registered Nurse (RN) license – Pennsylvania
• Acute care or Home Health experience strongly preferred (ICU, ED, Med/Surg, Stepdown, etc.)
• Strong clinical judgment and ability to work independently
• Excellent communication and patient education skills

Schedule & Compensation

Monday–Friday | 40 hours/week
Salary: $80,000 – $108,000 (depending on years of RN experience)
Mileage reimbursement for home visits
Up to $8,000 tuition reimbursement for continued education
