Case Manager Position
Job Summary
As a member of a multidisciplinary care management team, the nursing case manager is responsible for coordinating the care and services of selected patient populations.
Essential Responsibilities
- Comprehensively assess patient goals including biophysical, psychosocial, environmental, discharge planning needs, and financial status.
- Monitor patient progress for changes in medical needs affecting current and future treatment.
- Procure services and advocate on behalf of patients and families, especially when resources are scarce.
- Collaborate with third-party payers to resolve issues impacting continuity of care.
- Coordinate and direct patient discharge planning with family members, healthcare team, and community resources.
- Review admission data timely to determine appropriate level of care according to established criteria.
Required Skills & Experience
- Minimum 3 years clinical nursing experience in an acute care facility.
- Strong knowledge of nursing practices and procedures.
- Strong clinical assessment skills.
- Knowledgeable about community resources to support patient recovery.
- Understanding of third-party payer reimbursement and regulatory issues.
- Ability to explain complex technical information clearly to patients and families.
- Strong interpersonal skills for gathering information and assessing patient needs.
Contract Length
- Approximately 13 weeks (89 days rounded to nearest week).
