Location: Waterloo, Iowa
Employment Type: Travel
Contract Length: 13 weeks
Schedule
- Every third weekend

- Tdap (current, non-expired; declination accepted if no pediatric interaction)
- Physical/Health Statement (fit to work exam within last year)
- Trinity 10 Panel Drug Screen (completed within 30 days of start)

- Trinity I9 Form
- Confidentiality and Information System Security Agreement
- Integrity and Audit Services Form
- Non Employee Upload Form
- Specialty Specific Competency Testing (completed within last year)
- Criminal Background Check (Federal, Nationwide, County, last 7 years; dated within 30 days of start)
- Education Verification (highest level of academic education)
- EPLS (Excluded Parties List System) check (within 30 days)
- GSA/OIG Exclusion check (within 30 days)
- Joint Commission Testing (CORE Exam within last year)
