Physical Therapist

Location: Tuba City, Arizona
Employment Type: Contract
Contract Length: 13 weeks

Job Description

• Provide quality physical therapy services primarily to a Native American population.
• Manage and implement the Physical Therapy program.
• Perform physical therapy evaluation and treatment for inpatients and outpatients, from birth through geriatric patients.
• Provide specialty services including wound care, prosthetic and orthotic clinic, geriatric clinic, high school clinic, functional clinic, spinal cord clinic, assistive technology/adaptive equipment evaluation/treatment, and Durable Medical Equipment Prosthetic Orthotic Supply Program.
• Lead the physical therapy section including quality improvement, risk management, safety, policy and procedure development.
• Participate in Joint Commission preparation, staff education, reporting, staff meetings, and in-service training.
• Fulfill responsibilities listed on the Medical Staff Credentialing Core Privileges form.
• Work schedule includes Monday to Friday, 8am-5pm, with weekend rotation.

Required Qualifications

• Valid Rehabilitation Therapist license in Arizona.
• Minimum of 3 years of Rehabilitation Therapist experience, including experience within the last year.
• Experience as a Physical Therapist.
• Current Basic Life Support (BLS) certification issued by the American Heart Association.
• Must meet credentialing requirements to serve on the Medical Staff.
• Fingerprinting must be submitted and cleared prior to start.

Additional Information

• Orientation occurs every 2 weeks.
• COVID and flu vaccination exemptions may be accepted after submission of a declination form.
• Housing application must be submitted 2 weeks prior to start.
