POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS – NC/compact RN license; BLS; 2 years case manager experience in an acute hospital setting; Acute hospital experience during the past 12 months with no more than a 90-day break

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8:00AM - 4:30PM

SYSTEMS – CERNER, NATE, ENSOCARE

START DATE – ASAP
