The job position requires a variety of professional and personal health credentials for onboarding, including a Driver's License/State ID, name verification, and social security card for verification purposes. Drug screening, color-blind test, fit test, and a physical/health statement are also needed. A COVID-19 vaccine, MMR, hepatitis B, influenza vaccine, TB test, varicella, Tdap, and annual TB questionnaire are mandatory. A criminal background check and employment eligibility verification (Form I-9) are required as well. Key professional documents include a resume, two references, a criminal background check, education verification, and a Nursys licensure verification. For onboarding, the Prime Core Competencies Exam, Prime Skills Checklist, Prime Basic Pharmacology Exam, Prime Specialty Exam, and an Essentials Onboarding Packet are necessary. The Nursys license and certification are also specified. Submission requirements include the Prime Resume, Prime Reference #1 and #2, and the Prime Skills Checklist. All other credentials and exams are for onboarding. Additionally, on-call duties are required, with the schedule including rotating weekdays, weekends, and holidays.
