Case Manager

Overview

Travel/contract RN case management role in an acute-care hospital setting
13-week assignment
Day shift: 4x10-hour shifts per week
Rotating weekend schedule
Local candidates accepted

Required Qualifications

Valid RN license in Oklahoma
Acute Care Case Management experience within the last year
1 year of Acute Care Case Management experience
BLS issued by American Heart Association or American Red Cross
Associate of Science in Nursing / Associate Degree in Nursing
Driver's License
Experience with Epic

Preferred Qualifications

Experience with MCG or InterQual
CCM certification

Key Responsibilities

Manage complex hospitalized patients
Coordinate services and care transitions
Support discharge planning
Document accurately in the electronic medical record
Work independently and within a multidisciplinary care team
