How long is the contract assignment? • -13 weeks 2. Schedule of assignment hours? M – F, 8am – 5pm? -M-F; 0800-1700 3. Will this candidate be visiting patients in care facilities or only in patient’s homes? • - private residence, ALFs 4. Will you want to hire the contractor at the end of the contract or is this truly a contract assignment? - contract 6. What region are you in? Home health or Hospice? -Home Health 7. To which location will this person report for assignment, full address, Including the OU (Location Code) 3356- Amedisys Home Health Newnan 1825 Hwy 34 E Newnan, GA 30265 Coverage Area: Troup County, GA (Hogansville, LaGrange, West Point). County is southwest of care center. Please confirm above location is correct and list any other territories that are to be covered by the candidates: 9. How many miles per week will the consultant travel? -35-45 daily; 170-200 weekly/ Depends on contractor location 10. What are the weekly units of productivity expectations? • - 30 pts 11. What EMR systems will the consultant be using: • HCHB for documentation/OASIS, Forcura 12. Will there be any on call? If so, how much? • Yes; 1 weekend per month 13.
