You must have home health experience and a current California PT license. The caseload is 5-6 visits per day, and you will document using Kinzer EMR. Your responsibilities include providing skilled physical therapy in patient homes, developing individualized care plans, and collaborating with the healthcare team to support mobility and independence. Mileage is reimbursed at IRS $0.72 per mile. Alameda and Contra Costa counties offer diverse neighborhoods, scenic parks, and easy access to Bay Area attractions.
