StartDate: ASAP Available Shifts: 8 D Pay Rate: $2525.00 - $2602.00

Travel PT Home Health jobs in Queens, NY start ASAP and offer a 13-week contract with 8-hour days, Monday through Friday, and 5-6 visits per day, totaling 40 hours per week. You must have experience working with the geriatric population and hold an active New York PT license. Home health experience is preferred but not required. Your responsibilities include evaluating and treating geriatric patients in their homes, developing individualized care plans, and documenting all care using Homecare Homebase EMR. You will educate patients and families on mobility and safety, collaborate with interdisciplinary teams, and ensure patient progress toward therapy goals. No mileage reimbursement is provided.
