This Home Health Physical Therapist Assistant role is based in College Park, Georgia, a welcoming community just minutes from downtown Atlanta. In this position, you will provide one-on-one rehabilitative care to patients in their homes throughout College Park and surrounding communities. You will work under the supervision of a licensed Physical Therapist, implementing individualized care plans designed to help patients regain mobility, improve safety, and enhance independence in their daily lives. Your typical day will include traveling between patient homes, conducting therapy sessions, documenting care in the electronic medical record, and communicating updates to the interdisciplinary team. Daily responsibilities typically include performing therapeutic exercises and functional mobility training, assisting with gait and balance activities, educating patients and caregivers on home exercise programs, and evaluating home environments for safety considerations such as fall risks and equipment needs. You will reinforce patient-specific goals, monitor progress, and collaborate with the supervising Physical Therapist and other team members to adjust treatment plans as needed. Patient caseload commonly includes adults and older adults recovering from orthopedic surgeries, strokes, cardiac events, chronic conditions affecting mobility, and general deconditioning. Visit lengths are typically structured to allow focused, individualized care, enabling you to build strong therapeutic relationships and see the direct impact of your interventions. Patient ratios are designed to support quality time with each individual, rather than high-volume, rushed encounters. You will be part of a supportive, collaborative home health team that values open communication, ongoing clinical development, and shared problem-solving. The environment encourages autonomy during your day-to-day visits while maintaining strong connection with physical therapists, nurses, occupational therapists, and other disciplines for coordinated care. There are opportunities to refine your skills in specific clinical areas such as orthopedic, neurologic, or cardiopulmonary rehabilitation, making this a strong role for professional growth and future career advancement. Scheduling generally involves daytime hours with some flexibility, based on patient needs and agency requirements.
