This Home Health Physical Therapist opportunity is based in the beautiful coastal community of Santa Barbara, California and surrounding areas. Within this setting, you will provide in-home physical therapy to adult and geriatric patients recovering from surgery, illness, injury, or managing chronic conditions. You will visit patients in their homes across Santa Barbara and nearby communities, offering a meaningful opportunity to help individuals maintain independence and safely remain in their preferred living environment. Typical responsibilities include conducting comprehensive initial and ongoing assessments, developing individualized plans of care, setting functional mobility goals, and coordinating with a multidisciplinary home health team. You will perform gait and balance training, therapeutic exercises, transfer training, home safety evaluations, and patient and caregiver education. Your work may include a diverse caseload such as orthopedic post-acute cases, neurologic conditions, cardiopulmonary patients, and general deconditioning. A typical day includes traveling between patient residences, performing scheduled visits, documenting electronically, and communicating with the broader care team. Patient volume is managed to support quality one-on-one care while allowing reasonable travel time. You can expect to work with a generally moderate caseload, with visit frequencies determined by patient needs and physician orders. Shifts are usually daytime hours with flexibility built around patient scheduling and your route, making this role attractive for clinicians who value autonomy and schedule control within a full-time or near full-time framework. You will be supported by a collaborative and professional home health team with a strong emphasis on evidence-based practice, patient-centered care, and continuous quality improvement.
