Job Summary
Outpatient Physical Therapist (PT OP) This position is located at a healthcare provider's outpatient setting. The role involves evaluating patients, developing and implementing individualized treatment plans, and ensuring effective recovery and functional return to community activities. The therapist will serve as an educator for patients, families, and colleagues to promote optimal patient outcomes.

Job Details

• Position Description: Responsible for evaluating assigned patients, creating and modifying treatment plans based on patient needs, goals, and physician directives. Delegates and oversees discipline-specific therapy services, designs functional treatment programs, and fosters patient education. Ensures compliance with organizational standards and maintains professional conduct aligned with the organizations Mission Statement and Core Values. Promotes safety and adheres to organizational policies on patient handling, infectious disease control, and workplace safety.
• Required Qualifications: Degree in Physical Therapy from an accredited school; 1+ years of outpatient clinical experience; CPR certification; active license in the state where work will be performed.
• Schedule: Day shift, 8 hours per day, minimum of 40 hours per week.
• Additional Information: Completes comprehensive patient evaluations and produces clear, accurate medical documentation. Participates in student training and contributes to organizational performance improvement initiatives. Maintains licensure and adheres to state practice acts and regulatory standards. Demonstrates a commitment to professional growth through continuing education and self-directed learning.
