Description

Job Summary:
Occupational Therapist Home Health (OT HH) positioning in a home health setting. This role involves providing skilled therapy services to geriatric patients, conducting assessments, and implementing treatment plans in accordance with established standards. The position allows for flexibility within a specified territory, requiring timely documentation and communication to ensure quality patient care.

Job Details:

  • Schedule: 8-hour shifts, 40 hours minimum per week, starting on 10/05/2026 and ending on 01/04/2027
  • Required Qualifications: Graduation from an accredited occupational therapy program; active state license in hand; minimum of 1 year of relevant experience; recent experience completing 6 daily visits or a minimum of 30 points weekly for productivity; BLS certification; proficiency in Homecare Homebase (HCHB) for staff completing Start of Care (SOC); valid drivers license and personal transportation required; on-call rotation may be required; timely documentation and communication mandatory
  • Preferred Qualifications: Experience in home health and geriatric patient care
  • Additional Information:
    • Patient load is approximately 5-7 patients per day, with clinical responsibilities including activities of daily living, edema massage, electrical stimulation, feeding techniques, and managing orthopedic and soft tissue injuries.
    • Flexibility with territory within up to a 60-mile radius from the hub/office, with occasional travel beyond based on patient needs.
    • Equipment includes items such as dynamic splints, functional splints, upper extremity prosthetics, and specialized procedures.
    • No first-time travelers accepted; this role requires previous experience with home health.
