Description
- Assess client needs – Evaluate clients’ social, emotional, financial, and family situations and identify their needs and goals.
- Provide counseling and support – Offer emotional support, crisis intervention, counseling, and guidance to individuals and families.
- Connect clients with resources – Refer clients to healthcare, housing, financial assistance, mental-health services, and other community resources.
- Case management and documentation
- Years of experience: 2-year acute hospital case management experience required
MSW Certifications:
BLS/CPR Certification.
