Requirements

MSW required
LCSW preferred
Minimum 2 years of experience
Acute medical hospital Social Work experience required
Epic experience required
Travel experience required; first-time travelers are not accepted
Experience with assessment, care coordination, and discharge planning
Strong knowledge of advance directives, POLST, protective services, homelessness, mental health/crisis intervention, and substance-use treatment resources
Experience organizing/participating in family meetings and treatment planning
Knowledge of community resources including Medicaid, Medicare, Social Security, housing, food assistance, and medical transportation
Trauma Level I experience preferred
