Registered Nurse (Travel/Contract)

Location: Santa Rosa, California
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Requirements

Must have a valid California RN license and certifications in hand to submit
Minimum 2 years recent relevant experience required
Must have case management experience
Certifications required:
- Registered Nurse of California (RN)
- Basic Life Support Healthcare Provider (BLS)
- Valid California Driver’s License (DL)
- Auto Insurance (INS)
Locals accepted within 50 miles travel radius

Shift Details

Schedule: 5 days a week, 8-hour shifts (8:00 AM - 5:00 PM)
Weekend requirements apply

Job Description

Perform routine assignments and develop competence through structured work
Accountable for assessment, coordination, delivery, and evaluation of in-home nursing care
Provide direct patient care, patient/family education, and manage transitions of care
Deliver holistic, individualized care considering psychological, sociocultural, spiritual, economic, and lifestyle factors
Develop and manage an interdisciplinary plan of care
Foster collaborative relationships with patients, families, physicians, and healthcare providers through effective communication
Adhere to policies, industry standards, best practices, laws, regulations, and codes to ensure a quality patient experience
Engage in continuous professional growth and development in nursing practice
