Job Description
- Position: Registered Nurse (RN) in Intensive Care Unit (ICU)
- Facility: Ascension Health system in Wichita, KS
Required Credentials and Onboarding Requirements
Licenses & Certifications
- Current nursing license with primary source verification within 30 days prior to assignment start.
- Nursys verification within 30 days prior to start.
- Basic Life Support (BLS) - American Heart Association (AHA) certified.
- Advanced Cardiovascular Life Support (ACLS) - AHA certified.
- CPI/MOAB High Risk Online Portion (Nonviolent Crisis Intervention with Advanced Physical Skills or MOAB De-Escalation Course) completed prior to start.
- Annual NIH Certification (Group A yearly, Groups B-F every 2 years).
- Tdap vaccine valid within 10 years (no declinations accepted).
- Influenza vaccine for flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement.
- MMR: Proof of two vaccines or positive titer (no declinations).
- Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider (no declinations).
- Hepatitis B: Documentation of vaccination series completion or signed declination.
- Fit Test for respiratory protection (preferred before start, can be done onsite).
- 2 Step PPD or IGRA testing for TB within required timeframes; annual TB questionnaire.
- Ascension 12 Panel Drug Screen within 30 days prior to start and annually thereafter.
Professional Documentation
- Resume/Profile submission required.
- At least two professional references prior to first assignment.
- Criminal background check including Social Security Trace, Employment Verification, OIG Search, National Sex Offender Registry, and others within 30 days of assignment start.
- Employment verification for the past seven years.
- Education verification relevant to clinical work within 90 days of start.
- Driver’s License or State ID (must be current and match address requirements).
- Ascension Profile and other onboarding attestations and education modules.
Testing & Competency Exams (must be completed prior to or at onboarding)
- Ascension Cerner Test-Out (required for onboarding).
- Ascension Specialty Exam (minimum 80% passing score, updated every 3 years).
- Ascension Core Competencies (HIPAA, Joint Commission, OSHA topics; minimum 80% score, updated yearly).
- Ascension EKG Exam (minimum 80% passing score, updated as per latest edition).
- Ascension Skills Checklist (clinical specialty, updated annually).
- Ascension Onboarding Requirements Education Attestation.
- Ascension Nursing Contract Worker Onboarding (done onsite).
- Ascension Nova StatStrip pre-hire modules.
- Ascension Confirmation of Assignment documentation upon initial assignment and extensions.
- Ascension Employment Verification and EPLS (Exclusionary Provider List System) checks.
Health & Personal History
- Statement of good health or completed physical exam prior to first assignment, updated annually.
- Ascension Color Vision test completed prior to start and updated every 3 years.
- Positive PPD with negative chest X-ray and annual TB questionnaire (if applicable).
- Optional: Positive PPD documentation and chest X-ray within one year if applicable.
Optional / Additional
- Ascension Unit Specific Checklist (optional for onboarding, uploaded by unit leader).
- On-site portion of CPI/MOAB High Risk training (completed at orientation).
- Hiring Manager Evaluation (post-assignment).
- Legal documentation for name changes (if applicable).
