Personal Health History
- COVID-19 Vaccine: Proof of vaccine and 1 booster or approved exemption (optional).
- MMR: Proof of 2 immunizations or positive IGG titer; if negative, booster or declination accepted.
- Hepatitis B: Proof of 3 immunizations and positive antibody titer; if negative, booster or declination accepted.
- TB Test: Negative TB skin test, QuantiFERON Gold, or T-spot within 30 days of start; if positive, must provide history, chest x-ray within 5 years, and annual questionnaire.
- Health Statement: Signed statement within 1 year confirming ability to work without restrictions.
- Varicella: Proof of 2 immunizations, positive titer, or declination accepted.
- Tdap: Proof of vaccination within past 10 years or declination accepted.
- Influenza Vaccine: Proof of vaccine from October to March or declination accepted.
- Drug Screen: 9-panel drug screen (minus THC) within 30 days of start, updated annually; rapid drug screen accepted; retesting required if break in service > 30 days.
- Covid Test: Required on start date for clinical staff (optional).
Professional Credentials
- Driver's License/State ID: Required.
- License/Certificate Verification: Primary source verification of professional license (e.g., Nursys) within 30 days of start, updated upon expiration.
- Criminal Background Check: 7-year county and national background check, SSN trace, employment verification, national sex offender, OFAC, and VSOP completed within 6 months of first assignment; updated every 3 years.
- OIG Search: Manual search within 30 days of start, updated annually.
- EPLS (Excluded Parties List System): Manual search within 30 days of start, updated annually.
- OFAC (Office of Foreign Assets Control): Manual search within 30 days of start, updated annually.
- National Sex Offender Search: Manual search within 30 days of start, updated annually.
- Medi-Cal Suspended and Ineligible Provider List: Search within 30 days prior to first assignment and updated monthly thereafter.
- CHS Confirmation of Assignment/Job Description: Must be updated with each extension.
- CHS Acknowledgement of Receipt for Policy Packet: Must be signed and dated.
- CHS Dispute Resolution Agreement: Must be signed and dated.
- CHS Badge Photo: Required.
- CHS ROI for MyChart: Must be initialed, signed, and dated.
- CHS Any DNS (Do Not Rehire/Send): Attestation of any known DNS record from prior 3 years.
- CHS Education Verification: Diploma, degree, transcripts, or board certification with primary source verification prior to first assignment.
- CHS Skills Checklist: Clinical skill-specific checklist required within 1 year of start.
- CHS Confidentiality of Receipt: Must be signed and dated.
Certifications
- BLS: American Heart Association (AHA) or American Red Cross (ARC) only.
- ACLS: AHA or ARC only.
- NRP (Neonatal Resuscitation Program): AHA or ARC only.
- AWHONN/Fetal Heart Monitoring: Required.
