    Required Credentials & Compliance

    Personal Health History

    • COVID-19 Vaccine: Proof of vaccine and 1 booster or approved exemption (optional).
    • MMR: Proof of 2 immunizations, positive IGG titer, or booster/declination accepted (required).
    • Hepatitis B: Proof of 3 immunizations and positive antibody titer, or booster/declination accepted (required).
    • TB Test: Negative TB skin test, QuantiFERON Gold, or T-spot within 30 days of start; if positive, must provide chest x-ray and annual questionnaire (required).
    • Health Statement: Signed statement within 1 year confirming ability to work without restrictions (required).
    • Varicella: Proof of 2 immunizations, positive titer, or declination accepted (required).
    • Tdap: Proof of vaccination within past 10 years or declination accepted (required).
    • Covid Test: Required on start date for clinical staff (optional).
    • Influenza Vaccine: Proof of vaccine from October - March or declination (required).
    • Drug Screen: 9 panel (minus THC), within 30 days of start, updated annually; rapid drug screen accepted (required).

    Professional Credentials

    • Driver's License/State ID: Required.
    • License/Certificate Verification: Primary source verification of professional license within 30 days of start, updated upon expiration (required).
    • Criminal Background Check: 7-year county and national background check, SSN trace, employment verification, national sex offender, OFAC, and VSOP completed within 6 months of first assignment, updated every 3 years (required).
    • OIG Search: Manual search within 30 days of start, updated annually (required).
    • EPLS (Excluded Parties List System): Manual search within 30 days of start, updated annually (required).
    • OFAC (Office of Foreign Assets Control): Manual search within 30 days of start, updated annually (required).
    • National Sex Offender Search: Manual search within 30 days of start, updated annually (required).
    • Medi-Cal Suspended and Ineligible Provider List: Search and documentation within 30 days prior to first assignment, updated monthly (required).

    Certifications

    • BLS (Basic Life Support): AHA or ARC only (required).
    • ACLS (Advanced Cardiovascular Life Support): AHA or ARC only (required).
    • NRP (Neonatal Resuscitation Program): AHA or ARC only (required).
    • AWHONN/Fetal Heart Monitoring: Required.

    Professional Documentation

    • CHS Profile (Resume or Profile): Required for submission.
    • 2 Professional References: Minimum 1 supervisory reference required for submission.
    • CHS Skills Checklist: Clinical skill-specific checklist completed within 1 year of start (required for submission).
    • CHS Education Verification: Diploma, degree, transcripts, or board certification with primary source verification (required).
    • CHS Confirmation of Assignment/Job Description: Must be updated with each extension (required).
    • CHS Acknowledgement of Receipt for Policy Packet: Signed and dated by candidate (required).
    • CHS Dispute Resolution Agreement: Signed and dated by candidate (required).
    • CHS Badge Photo: Required.
    • CHS ROI for MyChart: Signed and dated by candidate (required).
    • CHS Confidentiality of Receipt: Signed and dated by candidate (required).
    • CHS Any DNS (Do Not Rehire/Send): Attestation of any known DNS record from prior 3 years (required).

    Additional Notes

    • Fit Test will be completed onsite on start date.
    • Some credentials are optional or conditional based on prior documentation or timing.
    • All required documentation must be current and compliant with specified timelines.

    This position is ideal for experienced RN Labor and Delivery nurses who meet the credentialing and compliance requirements and are interested in a travel assignment in California.
