Travel RN Medical/Surgical Position - Wisconsin (Chilton)

Job Details

  • Experience Required: Minimum 2 years Medical/Surgical experience
  • Additional Experience: Rural healthcare experience is a plus but not required
  • Systems: EPIC experience required
  • Work Schedule: Every Other Week (EOW) and Every Other Holiday (EOH)
  • Floating: May need to float onsite within skill set or to nearby hospitals

Required Certifications & Licenses

  • BLS: Must be American Heart Association (AHA) certified
  • Nursys Verification: Within 30 days prior to start; disciplinary actions require FACIS Questionnaire
  • Current License & Primary Source Verification: Within 30 days prior to start; disciplinary actions require FACIS Questionnaire
  • Driver's License/State ID: Must be current and valid; address must match licensing requirements

Required Exams & Competencies

  • Ascension Color Vision Test: Completed prior to start and updated every 3 years
  • Ascension Core Competencies: HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start and updated yearly; minimum 80% score, max 2 attempts
  • Ascension EKG Exam: Must pass with at least 80%; max 2 attempts
  • Ascension Specialty Exam: Completed prior to first assignment and updated every 3 years; minimum 80% or national benchmark passing score; max 2 attempts
  • Ascension Epic Test-Out: Epic test results and training checklist with signed attestation required
  • Ascension Skills Checklist: Clinical skills checklist to be completed within one year of start and updated annually

Onboarding & Training Requirements

  • Ascension Onboarding Requirements Education Attestation: 6 hours pre-hire training with attestation
  • Ascension Nursing Contract Worker Onboarding: Completed onsite
  • Ascension Nova StatStrip: Pre-hire modules for non-allied positions
  • Ascension Unit Specific Checklist: Optional but needs to be uploaded within a week by unit leader

Health & Immunization Requirements

  • MMR: Documented proof of two vaccines or positive IGG titer; no declinations accepted
  • Hepatitis B: Vaccination series completion or signed declination
  • Influenza Vaccine: Required for flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
  • Positive PPD/Negative Chest X-Ray & Annual TB Questionnaire: Must provide positive IGRA or PPD results, chest x-ray within a year, and completed TB questionnaire; updated annually
  • 2 Step PPD OR IGRA: Required within 30 days prior to start
  • Varicella: Proof of two vaccines, serological immunity, or history verified by healthcare provider; no declinations accepted
  • Tdap: Valid within 10 years; no declinations accepted
  • Fit Test: Required for all travelers; preferred prior to start, can be completed onsite; specific mask types required for WI and MI; updated annually
  • Health Statement: Statement of good health or physical exam prior to first assignment; updated annually
  • Ascension 12 Panel Drug Screen: Completed within 30 days prior to start; includes 12 specified drugs; must include MRO review if applicable

Professional Documentation & Verification

  • Ascension Profile (Resume): Required for submission
  • Professional References: At least two qualifying references prior to first assignment
  • Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check; completed within 30 days of first assignment; updated annually or after break in service > 90 days
  • Employment Verification: Third-party verification for past 7 years; updated annually or after break in service > 90 days
  • Education Verification: Verification of relevant education within 90 days of start; updated if break in service > 90 days
  • Confirmation of Assignment: Provided upon initial assignment and updated with extensions
  • Ascension EPLS (Excluded Parties List System): Run within 90 days of start; rerun if break in service > 90 days

Additional Notes

  • Floating may be required within skill set or to nearby hospitals.
  • Rural healthcare experience is a plus but not mandatory.
  • All required documentation and verifications must be current and compliant with Ascension Health policies.
