Job Opportunity: RN Case Manager

Location: Northampton, Massachusetts

Contract Length: 26 weeks (approximately 6 months)


Work Schedule

  • Monday – Friday, 8:00 AM – 4:30 PM
  • Weekend rotation: 1 out of every 4 weekends

Job Description

  • Provide skilled nursing care and case management to patients and families.
  • Work under the direction of the Clinical Team Manager within established policies and procedures.
  • Deliver care in accordance with the State Nurse Practice Act and professional standards.
  • Utilize nursing theory and processes for patient care.
  • Communicate and document observations and assessments.
  • Promote and maintain health through direct care, teaching, counseling, and rehabilitative measures.

Required Credentials & Compliance

Licenses & Certifications

  • Professional Nursing License: Primary source verification required within 30 days of start; MA Conditional Licenses not accepted.
  • Basic Life Support (BLS): Required before onboarding; AHA preferred (also accepts Red Cross).
  • Driver’s License/State ID: Required before onboarding (front of card with signature if applicable).

Health & Immunizations

  • Fit Test (N95 Respirator): Required within 365 days of start and updated annually; must be completed prior to start.
  • MMR Vaccine or Immunity Proof: Two doses of measles, mumps, and rubella vaccines or proof of immunity.
  • Influenza Vaccine: Required during flu season (Sept 15 – April 30); no exemptions allowed.
  • TB Test: Documentation of TB skin test or negative blood test within 3 months of start; updated if break >6 months.
  • Health Statement: Physician-signed statement within 365 days of start; updated if break >6 months.
  • Optional but Preferred: Hepatitis B vaccine series or immunity proof; Tdap vaccine within 10 years.
  • Positive PPD: If applicable, negative chest x-ray documentation required.

Background & Verification Checks

  • Criminal Background Check: Within 45 days prior to start; includes county and international searches if applicable.
  • Massachusetts CORI Check: Within 30 days prior to start; updated if break >6 months.
  • MVR Check: Required for all home care positions; run within 45 days of start.
  • SSN Trace: Must be run with criminal check covering last 7 years.
  • National Sex Offender Registry Check: Within 30 days of start; updated annually.
  • Exclusion Checks: Includes OIG, GSA, MassHealth, and NH Medicaid lists; run within 30 days of start and annually.
  • Education Verification: Verified through background check company; highest level of education confirmed.

Documentation Required for Submission

  • Resume including past 7 years of work experience with city and state.
  • Two recent references (minimum one supervisor or above) in the relevant skillset.
  • Skills checklist specific to the role.
  • Candidate submission coversheet (current version).

Additional Onboarding Requirements

  • MGB badge photo (color photo taken within past 6 months, plain background, no filters or eyeglasses).
  • Respirator Medical Clearance Attestation (part of fit test record).
  • Confirmation of Assignment/POI form upon acceptance and for any assignment changes.
