Job Description

Required Credentials & Certifications

Licenses & Certifications

• BLS (Basic Life Support) – Must be American Heart Association (AHA) certified
• Nursys Verification – Within 30 days prior to start; disciplinary actions require FACIS Questionnaire
• Current Nursing License – Primary source verification within 30 days prior to start; disciplinary actions require FACIS Questionnaire
• NIH Certification – Group A updated yearly; Groups B-F updated every 2 years
• MOAB Moderate Risk De-Escalation Training
• Ascension Core Competencies – HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start, updated yearly, minimum 80% score
• Ascension EKG Exam – New edition as of Dec 1, 2023; passing score of 80% required with only two attempts
• Ascension Cerner Test-Out – Cerner test results and training checklist with signed attestation

Personal Health & Immunizations

• Ascension Color Vision Test – Completed prior to start and updated every 3 years
• MMR (Measles, Mumps, Rubella) – Documented proof of two vaccines or positive IGG titer; no declinations accepted
• Hepatitis B – Documentation of vaccination series completion or signed declination
• Influenza Vaccine – Required for flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
• Positive PPD / Negative Chest X-Ray and Annual TB Questionnaire – Positive IGRA or PPD results, chest x-ray within a year, TB questionnaire completed and updated annually
• 2 Step PPD OR IGRA (Quantiferon/Tspot) – Within 1 year and 30 days of start; TB questionnaire updated annually
• Varicella – Proof of two vaccines, serological immunity, or history verified by healthcare provider; no declinations accepted
• Tdap – Valid for 10 years; no declinations accepted
• Fit Test – Required for all travelers; preferred prior to start, can be completed onsite; updated annually
• Health Statement – Statement of good health or physical exam prior to first assignment, updated annually
• Ascension 12 Panel Drug Screen – Within 30 days prior to start; includes testing for 12 specified drugs; retesting required if break in service > 90 days

Professional & Employment Verification

• Ascension Profile (Resume)
• Two Professional References – Completed prior to first assignment
• Criminal Background Check – Includes SSN trace, employment verification, OIG search, national sex offender check; completed within 30 days of first assignment and updated annually or after break in service > 90 days
• EPLS (Excluded Parties List System) Check – Within 90 days of start; rerun if break in service > 90 days
• Education Verification – Relevant to clinical work; completed within 90 days of start; rerun if break in service > 90 days
• Employment Verification – Third-party verification for past 7 years; updated annually or after break in service > 90 days
• Ascension Confirmation of Assignment – Provided upon initial assignment and updates with extensions
• Driver’s License/State ID – Must be current, match address on file, and comply with compact license rules if applicable

Onboarding & Training

• Ascension Nursing Contract Worker Onboarding – Completed onsite prior to start
• Ascension Onboarding Requirements Education Attestation – 6 hours pre-hire training with attestation
• Ascension Nova StatStrip Training – Pre-hire modules for non-allied positions
• Ascension Skills Checklist – Clinical skills checklist completed within one year of start and updated annually
• Ascension Badge Photo – Forward-facing headshot with white or blue background, no alterations

Optional Credentials

• Ascension Unit Specific Checklist – Optional for onboarding, must be uploaded within a week
• Hiring Manager Evaluation – Post-assignment evaluation
• Ascension Name Change Document – Legal documentation of name change
