Home Health / Hospice RN - Wisconsin (Beaver Dam)

Contract Length

• 13 weeks

Required Certifications

• BLS (AHA)

Patient Population & Diagnoses

• Primarily geriatric patients (no pediatrics)
• Common conditions include:

  • Heart Disease
  • Chronic Respiratory Diseases (CHF, COPD, Pneumonia)
  • Dementia/Alzheimer’s
  • Depression
  • Stroke
  • Cancer
  • Kidney Disease
  • Diabetes
  • Liver Disease
  • Wounds
  • Joint Replacement
  • Surgery
  • Medication Management

Job Duties & Meetings

• Daily 15-minute huddle call
• Weekly interdisciplinary team (IDT) meeting at office
• Monthly all-staff meeting
• Utilization review involvement

Equipment

• No ventilators
• Occasional CPAP & BiPAP (patients self-manage)

Experience & Skills Required

• Travel nursing experience
• Critical thinking abilities
• Chronic disease management
• Patient and family education
• Negative Pressure Wound Therapy (NPWT)
• Ostomy care
• Central line care
• IV administration
• Complex wound care
• Hospice experience including pain and symptom management, end-of-life care, empathy, and compassion

Patient Load

• 5-6 patients per day depending on visit type
• 25-30 points per week
• Routine and OASIS visits

Supervision

• Administrator available after hours, weekends, and holidays

Support Staff

• 3 certified nursing assistants available

Schedule & Shifts

• Monday through Friday, 8:00 AM to 4:30 PM (end time may vary)
• Weekend work only during on-call shifts
• Must be available to work at least 1 major holiday and 1 minor holiday
• On-call requirement: 1 week/weekend every 6 weeks (days do not have to be consecutive)
• No floating between departments, but will cover both home health and hospice visits

Dress Code

• Solid color scrubs (no patterns or logos unless company-issued)
• Closed-toe, non-slip shoes with socks or hosiery
• No flip-flops, open-toe shoes, or sandals in patient care areas

Orientation

• 1 day computer-based training
• 2-5 days field training/shadowing depending on experience

Additional Notes

• Clinicians must clock in and out at the facility
• Previous employment with the facility requires a minimum 6-month gap before reapplying
