Job Description

Required Credentials & Onboarding Requirements

- Ascension Resume
- Nursys license verification (within 30 days prior to start)
- License/Certificate verification (current and primary source within 30 days prior to start)
- Driver's License/State ID (valid, not expired, matching address requirements)
- Ascension Profile (resume or profile)
- Two professional references (qualifying, prior to first assignment)
Onboarding Requirements (Must be completed prior to or at start)
- Signed Assignment Agreement (offer letter, initial confirmation, and extension)
- BLS Certification (American Heart Association required)
- Ascension Color Vision test (completed prior to start, updated every 3 years)
- Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics; minimum 80% score, two attempts allowed)
- NIH Certification (Group A yearly, Groups B-F every 2 years)
- Ascension EKG Exam (passing score of 80%, two attempts allowed; updated exam as of Dec 1, 2023)
- Ascension Onboarding Education Attestation (completion of 6-hour pre-hire training)
- Ascension Nursing Contract Worker Onboarding (completed onsite)
- Ascension Nova StatStrip pre-hire modules (required for non-allied positions)
- MOAB Moderate Risk De-Escalation Training
- Ascension Cerner Test-Out (test results and training checklist with signed attestation)
- MMR Vaccination (proof of two vaccines or positive IGG titer; no declinations accepted)
- Fit Test (required for all travelers; preferred prior to start, can be onsite; updated annually)
- Hepatitis B Vaccination or signed declination
- Influenza Vaccine (required during flu season; exemptions accepted with mask requirement)
- Health Statement or physical exam (completed prior to first assignment, updated annually)
- 2 Step PPD or IGRA testing (within specified timeframes; TB questionnaire updated annually)
- Varicella Immunity (proof of two vaccines, serological evidence, or history; no declinations accepted)
- Tdap Vaccine (valid within 10 years; no declinations accepted)
- Ascension Criminal Background Check (including SSN trace, employment verification, OIG search, national sex offender registry; updated annually or after break in service)
- Ascension EPLS check (within 90 days of start; rerun if break in service >90 days)
- Ascension Skills Checklist (clinical skills, completed within one year of start, updated annually)
- Ascension 12 Panel Drug Screen (within 30 days prior to start, updated annually; retesting if break in service >90 days)
- Education Verification (relevant to clinical work, completed within 90 days of start; rerun if break in service >90 days)
- Confirmation of Assignment (provided upon initial assignment and updates with extensions)
- Employment Verification (past 7 years, third-party; updated annually or after break in service)
Optional Credentials
- Ascension Unit Specific Checklist (optional for onboarding; uploaded by unit leader within a week)
- Positive PPD with Negative Chest X-Ray and Annual TB Questionnaire (optional if applicable)
