    Registered Nurse (Travel/Contract) - Novato, California

    Job Summary

    Seeking an experienced Registered Nurse with home health experience as a Case Manager. This role involves overall management of patients in a home health setting, including direct care, patient/family education, and coordination of interdisciplinary teams to ensure quality care and effective visit utilization.

    Note: Recent home health experience is required; hospital case management experience is not applicable. Locals living near the facility are highly preferred (within 35 miles).

    Key Responsibilities

    • Manage and coordinate in-home nursing care for assigned patients.
    • Conduct comprehensive patient and family assessments (physiological, spiritual, emotional).
    • Collaborate with physicians and healthcare team to develop and adjust patient-centric care plans.
    • Provide compassionate, evidence-based nursing care in compliance with policies and regulations.
    • Educate patients and families on care plans and treatments using teach-back methods.
    • Maintain a safe and therapeutic environment for patients and families.
    • Delegate tasks appropriately to team members within their scope of practice.
    • Monitor and evaluate patient responses to care plans; adjust as necessary.
    • Document all patient care and education accurately in the electronic health record (EHR).
    • Report safety or compliance concerns and participate in safety event analysis.
    • Maintain confidentiality of patient health information (PHI).
    • Engage in continuous professional development and stay current with best practices.

    Required Certifications

    • California RN License
    • Basic Life Support (BLS) from American Heart Association (AHA) or American Red Cross (ARC)
    • Experience with OASIS documentation is mandatory

    Contract Length

    • 13 weeks

    Background and Compliance Requirements

    • 7-year background check including SSN trace, county searches, sex offender search, education verification, and FACIS Level 3 screening.
    • Motor Vehicle Report required (due to driving responsibilities).
    • Annual updates required for EPLS/GSA/SAM, HHS/OIG, and FACIS screenings.
    • Education verification required (Bachelor of Science in Nursing preferred).
    • Drug screening (9-panel, excluding marijuana/cannabinoids due to CA law).
    • Negative TB test or chest x-ray within 90 days of start; annual renewal required.
    • Immunizations and titers required: TDAP, Measles, Mumps, Rubella, Hepatitis B, Varicella.
    • Physical exam within 1 year of start.
    • Fit mask test within 1 year, renewed annually.
    • Seasonal flu vaccination required during flu season (Nov 1 - Apr 30).
    • Respiratory safety clearance required prior to start and annually.
    • References and full 7-year work history required.

    • Must live locally (within 35 miles).
    • Position requires driving; personal or auto insurance must be valid for the assignment duration.
    • Case management duties focus on home health, not hospital case management or hospice.
    • Emphasis on holistic, individualized patient care considering psychological, sociocultural, spiritual, economic, and lifestyle factors.
    • Strong communication skills required to foster collaboration among patients, families, physicians, and healthcare providers.
